Intake Process

Welcome to Dragonfly! We are excited to begin our work together. This page contains all the information you need to schedule your first intake appointment with your therapist. If you have any questions, you can always reach out to our administrative staff at admin@dragonflybrunswick.com.

Your first step is to request an appointment via our secure client portal. You can do this without a portal account. You can choose a particular therapist or choose the first available appointment with anyone. Also please choose whether you prefer in-person services or telehealth. If you would like in person counseling, we do ask for the safety of our staff and clients that you are fully vaccinated for COVID-19.

When we receive your request, we will set you up with a portal account where you can fill out your intake paperwork. You will receive an email from therapyportal.com with a link to the portal. Your initial intake packet should look like this:

  • Informed Consent for your therapist
  • Our Privacy Practices
  • Either a telehealth consent form or a consent for in-person services
  • Client History Form
  • Emergency Contacts
  • Payment Authorization Form
  • Insurance Form
  • An assessment form regarding your current symptoms

After your paperwork is complete, we will check your insurance benefits and confirm your intake appointment. You will receive a confirmation email with this information as well as information about how to access your appointment. If you do not receive this email, your appointment is not confirmed.

To reduce the chance that emails will go into spam folders, please add admin@dragonflybrunswick.com and appointmentreminders@therapyportal.com to your address book.
